How your Medical Coverage works

As a UnitedHealthcare medical plan member, you have all the tools and resources you need to take charge of your health. 

Your Medical Plan Options

Plexus offers 2 high deductible health plans that include a tax-advantaged health savings account (HSA), as well as a traditional medical plan:

  • Consumer Choice 2000
  • Consumer Choice 4000*
  • Surest

Your Deductible

You pay 100% until your deductible is met. Network preventive care is covered 100% and not subject to the deductible. The Surest plan does not have a deductible.

Your Coinsurance

Once you meet your deductible, the plan will begin to pay a percentage of your eligible healthcare costs; you and the plan will share the expenses. The Surest plan does not have coinsurance.

Your Out-of-Pocket Maximum

Once you reach your out-of-pocket maximum, the plan will pay 100% of eligible expenses for the rest of the year.

Your HSA

If you enroll in a Consumer Choice plan, Plexus will contribute $650 for individual coverage or $1,000 for family coverage. Company contributions are made on a per-paycheck basis and will be pro-rated. Be sure to elect the HSA in Workday in order to receive the Plexus contribution.

*If you enroll in the Consumer Choice 4000 medical plan, you will automatically be enrolled in Basic Critical Illness insurance at no additional cost** based on your medical coverage level.
**Plexus pays the premium, but will withhold tax for the premium amount to ensure any benefit received is tax-free. This will appear on your paycheck as Critical Illness imputed income.

In a Consumer Choice plan, your family’s combined health care costs are used to meet one deductible and out-of-pocket maximum.

In the Surest plan, this works differently as the plan has no deductible that needs to be met. Only an out-of-pocket maximum applies. The family out-of-pocket maximum is $8,000 with each family member having an individual out-of-pocket maximum of $4,000 in network.

Healthcare can get expensive. Fortunately, there are some simple things you can do to keep costs down and get the care you need (without sacrificing quality).

  1. Get Preventive Care
    Take advantage of 100% coverage when you use network providers for annual physicals, immunizations and preventive care.
  2. Stay in the Network
    You’ll always pay less when you use UnitedHealthcare network providers.
  3. Use Premium Care Physicians
    These doctors are known for delivering quality, cost-effective care.
  4. Compare Costs
    Avoid unexpected surprises. The amount you pay for care depends on the provider you choose. Compare costs on the member website before you get care, so you know what to expect.
  5. Know Where to Go for Care
    In a true emergency, call 911 or go to the nearest emergency room. For everything else, you’ll save time and money by exploring your options on the member website. To find care and compare costs, visit your member website or call Member Services.
