Maternity, Parenting and Menopause Support

What you eat and drink, how often you exercise and how well you follow any medications your doctor has recommended all play a big part in managing diabetes. To stay on track, take advantage of the Diabetes Management Program, available to you at no additional cost as part of your benefits. You’ll work one-on-one with a registered nurse who will help you manage your condition and support you in understanding and following your doctor’s treatment plan.

If you or a loved one is living with a chronic condition, a nurse can help you manage your symptoms and keep your health condition in check. When you enroll in the Condition Management Program, you’ll be connected with specially trained nurses who can provide support to you or a loved one to keep the following chronic conditions in check:

  • Asthma
  • Diabetes
  • Coronary artery disease (CAD)
  • Congestive heart failure
  • Chronic obstructive pulmonary disease (COPD)

The UnitedHealthcare Cancer Support Program (CSP) provides compassionate guidance and answers for you or a family member who’s faced with cancer. This resource is available at no additional cost to you as part of your health plan benefits. Assistance includes:

  • A nurse can help navigate treatment options and find a network provider from a high-quality Centers of Excellence (COE) facility.
  • Help finding answers to questions, and managing symptoms and side effects
  • Support working with your doctors, so you feel informed to make decisions for your health
  • Access to digital tools to help provide real-time guidance and identify care needs immediately

Make sure you’re receiving quality care and guidance when you need it most. The Centers of Excellence program refers you to quality providers that undergo a comprehensive annual review. These providers have demonstrated lower treatment costs and a higher likelihood of improved health outcomes in areas such as:

  • Cancer treatment
  • Transplant services
  • Bariatric surgery
  • Infertility
  • Musculoskeletal conditions
  • And many others

Need medical treatment or a recommended procedure but not sure where to go? Decision Support experts will work with you to understand your options, find providers, estimate out-of-pocket costs and more, so you can get the care that’s right for you.

If you need a transplant, this program can connect you to a transplant Center of Excellence. Experienced nurses will work with you and your doctors throughout your care.
