Separation of Employment
When you leave the company, you may have questions about pay, benefits and what you need to do. View the chart below to help guide you through the transition. For details, see your Human Resources representative or contact the appropriate resource, listed below.
Your core health benefits (medical, dental and vision) end on the last day of the month your employment ends. All other benefits end on your last day of employment. When your benefits end, you may choose to continue health benefits under COBRA continuation, while other benefits may be converted to personal policies by contacting the benefit carrier directly. Notifications on extending your benefits will be mailed to you. Please allow 2 weeks after your termination is processed to receive this information.
COBRA continuation is administered by UnitedHealthcare. For questions regarding enrollment or payment, please contact 1-844-210-5456.
