Unpredictable, life-changing events such as cancer, a car accident, a heart attack or an extended hospital stay can be personally and financially overwhelming. To help protect you during a financial hardship such as these, Plexus offers voluntary benefits, administered by Voya Financial.

If you are like most people, you don’t budget for accidents. You hope it never happens, but if it does, wouldn’t it be nice to have an insurance plan that pays benefits regardless of other insurance you may have? Accident Insurance pays you benefits for specific injuries and events resulting from a covered accident — regardless of other medical or disability benefits you receive. Benefit amounts depend on the type of injury and treatment, and may be paid out multiple times per person, per year (there’s no benefit cap).

Accident Insurance is designed to supplement your primary health insurance—not replace it.

This coverage provides additional financial protection if you or a covered family member is diagnosed with a serious illness, such as heart attack, stroke, organ failure or cancer. You have the option to purchase a plan that pays a lump-sum benefit of $5,000; this benefit would be paid if you or a covered family member is diagnosed with a covered illness or disease.

If you enroll in the Consumer Choice 4000 medical plan, you will automatically be enrolled in Basic Critical Illness Insurance at no additional cost* based on your medical coverage level. You have the option to elect an additional $5,000 in coverage. This benefit is not a replacement for your medical plan insurance (whether through Plexus or another employer). Critical Illness Insurance is considered a “limited benefit policy”, meaning it is not medical insurance and does not satisfy the requirement of minimum essential coverage.

* Plexus pays the premium, but we will withhold tax for the premium amount to ensure any benefit received is tax-free. This will appear on your paycheck as Critical Illness Imputed Income.

Hospital Indemnity Insurance provides a fixed daily benefit payment if you have a covered stay in a hospital, or critical care unit or rehabilitation facility beginning on or after your coverage effective date. You have the option to elect Hospital Indemnity Insurance to meet your needs. Hospital Indemnity Insurance is a limited benefit policy. It is not health insurance and does not satisfy the requirement of minimum essential coverage under the Affordable Care Act. For detailed information, view the Hospital Indemnity flyer below.

Features of Hospital Indemnity Insurance include:

  • Guaranteed issue: Pre-existing condition exclusion may apply.
  • Flexible: You can use the benefit payments for any purpose you like.
  • Portable: If you leave your current employer or retire, you can take your coverage with you and select from a variety of payment plans. (Portability provisions vary by state.)

  • Medical expenses, such as deductibles and co-pays
  • Travel, food and lodging expenses for family members
  • Childcare
  • Everyday expenses like utilities and groceries

Health screening tests covered by the Wellness Benefit include, but are not limited to:

  • Annual physical exam (adult)
  • Biometric screenings
  • Colonoscopy
  • Mammography
  • Routine eye exam
  • Routine dental exam
  • and more!

You can file an online wellness claim for each covered person with Voya, and you can receive your benefit by check or via direct deposit.
