   • Rate Changes
     • Surest – Decreasing 10.0% from CMP rates, -$23 to -$68 per month*
     • Consumer Choice 2000 – Increasing 15.0%, $19 to $65 per month*
     • Consumer Choice 4000 – Increasing 7.0%, $5 to $19 per month*
       *Approximate dollar decrease/increase depending on coverage level.
   • Surest Health Plan
     • The Surest health plan is a copay designed plan with no deductible or coinsurance that offers the ability to:
       • Shop and find cost and coverage information before making an appointment.
       • See provider ratings and reviews options. The lower copays are assigned to providers evaluated as higher-value.
       • Choose what works for you and your family.
       • The Surest plan uses the large, national UnitedHealthcare provider network.

5. Enroll in Benefits in Workday.
   Submit your Open Enrollment elections in Workday by 07 NOV. Please reference the enrollment instructions here. Be sure to elect your Life and AD&D insurance beneficiaries!
